To ensure the privacy of all patients, we store medical records in accordance with all federal laws relating to privacy and confidentiality, including the Health Insurance Portability and Accountability Act (HIPAA).
During your treatment at William P. Clements Jr. University Hospital, we will collect and store your health information on-site for two years. Any archived records will be stored off-site.

Please note: UT Southwestern Medical Center physicians/providers are not the custodian of the medical records. All requests for medical records need to be directed to the Health Information Management – Release of Information Department. There is a fee for processing your medical records request; however, there is no fee for sending records directly to another health care facility or physician for continuation of care.
